Physical Therapist – Full-time

Visiting Nurse & Hospice of Litchfield County is looking for a Physical Therapist to make home health visits in Litchfield County. The physical therapist provides preventive, restorative and physical therapy for patients. The referral for physical therapy is based on medical, nursing, social and therapeutic information, with the primary goal being to enhance the patient’s quality of life. The physical therapist may function as a case manager. The physical therapist reports to the Rehab Therapy Supervisor. Full benefit package, supportive work environment.
