Visiting Nurse & Hospice of Litchfield County is seeking EXPERIENCED HOME CARE RNs to join our progressive team of nurses, therapists, social workers, and home health aides. The agency adheres to National Best Practice Guidelines for caseloads and visit schedules. We offer a flexible work environment and a competitive benefits package. Weekend and holiday rotation.
Experience:
Acute Care or Extended Care: 1 year minimum preferred
Home Health Care: 1 year minimum preferred
License: Registered Nurse with valid CT RN License
RN Case Manager Positions Available (Full Time 37.5 hours per week)
- Assumes primary responsibility, including case management, for a patient caseload: this includes assessment, planning, implementation, teaching, and evaluation of all phases of the skilled nursing process
- Makes independent nursing decisions, utilizing a broad base of nursing practice standards and community resources to establish a plan of care applicable to the home setting, in collaboration with the patient, family or designated caregiver
- Coordinates and delegates patient care functions to other agency personnel while retaining primary care responsibility
